Notice of Privacy Practices
We may use certain information (name, address, telephone number, dates of service, age, and gender) to contact you in the future to raise money for St. Anthony Hospital and its affiliates. The money raised will be used to expand and improve the services and programs we provide to the community. If you do not wish to be contacted for fundraising efforts, please notify the foundation by e-mail at saintsfoundation@ssmhc.com or in writing to the following address:

The Notice of Privacy Practices will be publicized in the following ways:
- On the Foundation web page.
- In the Foundation’s Saints News publication.
- It will be available at the Foundation office.
Access to Patient Information
Currently, the Foundation does not access any patient records, but in order to ensure satisfaction of donors who have been admitted to our hospital, we do review a daily report, which includes only the following information: name, age, gender, address, other contact information, and relationship to hospital (donor, board member, employee, etc).
Opt Out Clause
An opt-out clause will be included in all solicitations used to contact potential donors, whether they are former patients or community members. The clause will contain the following language: If you wish to have your name removed from the list to receive this communication or future fundraising requests supporting St. Anthony Hospital, please e-mail the foundation at saintsfoundation@ssmhc.com or write to the following address:
